IF YOU ARE NEW to PCA, or to this scheduling software, you will be asked for a $20 deposit to hold your appointment. This will be applied to your treatment fee, or if applicable, to our late cancellation fee.

If it’s been more than 5 years since your last visit please schedule a new patient appointment. If you’re scheduling for yourself and someone else please enter THEIR name into our system so that they can complete their forms before their appointment time. Thanks!!!

If you have COLD, FLU or COVID symptoms, or you have been in close contact with someone who does, please STAY HOME until you’re better.

MASKING is encouraged but not required, unless you have any respiratory symptoms (even mild ones) like sniffling, sneezing, runny nose.

CANCELATION POLICY: By booking on our website, you agree to the following:

Appointments canceled with less than 12 hours notice may be charged a $20 cancelation fee. This fee will be invoiced and charged to your account using the card you have on file with us. This policy applies to all appointments.

Late cancelations mean other people miss out on a chance to get treated and impact clinic sustainability. We know things come up sometimes, and appreciate if you can let us know when they do. Thanks.
